How to format form 4 index HESLB Loan Application System (OLAMS)
- Your form four index number is used by the Board to uniquely identify applicants and must be formatted as follows. For example the index number S0332/12 of 2016 becomes a 15 character represented as S0332.0300.2016
- N.B: Overseas Applicants should use their examination index numbers as they appear in their certificates.

- Get number of Kumbukumbu ya Malipo/Control Number (99111xxxxxxx) after registration for paying TZS 30,000.00 kwa mkupuo mmoja through the following ways MPesa, AirtelMoney,Halopesa, T-pesa, Ezypesa au TigoPesa kwa kuchagua malipo ya Serikali.
- After making payments, utatumiwa stakabadhi yako kwa njia ya ‘sms’; Au, Tumia Namba ya Kumbukumbu ya Malipo/Control Number uliyopata kutoka katika mtandao wa OLAMS kufanya malipo katika Tawi lolote au Wakala yeyote wa Benki za TPB, CRDB na NMB. Ikiwa una swali, muulize Afisa wa Benki.
